In consideration of my application for entry into the Relay For Life event being accepted, I understand, intending to be legally bound for myself and my heirs, executors and administrators, waive and release the organisers and sponsors (individually and collectively), including the directors, officers, staff, volunteers and representatives thereof, and indemnify them against any liability (including liability for negligence) for the death or any physical or mental illness, incapacity or property damage or loss I may suffer which may directly or indirectly result from my participation in the event. I further verify that I am in proper physical and mental condition to participate in the event and acknowledge that I am aware of the risks involved and voluntarily agree to assume those risks.

I verify that the information supplied by me is true; I have not used a false name or impersonated another, nor used profanity, obscenities or offensive or defamatory remarks and I agree that The Cancer Council accepts no responsibility for any inaccuracies in the information I have provided. I understand that some of the details I provide here will be displayed on this site for the public and other members to view. I also agree that The Cancer Council accepts no responsibility for damage caused by automated emails resulting from my on line entry.
